在上述悖论之后还隐藏着另外一个悖论:即绝大多数卫生研究主体是富裕国家,但中低收入国家才是绝大部分全球公共卫生负担的承受者。这样的资源分配方式严重低效甚至违反道德,对需求最迫切民众的医疗方案研究起到了破坏作用。

可以肯定,靠从富国到穷国的直接资本和方案转移解决第一代全球发展问题是可能的。这样的例子包括小学入学促进计划,以及公共卫生领域的大规模免疫运动。
