  I call this "focus-inversion" , and hierarchical info filtering".I have seen this many times before in my departement ( Defense) and these are all "management-f*ckups" (mind the expression) , These issues will only be contained when we drasticly change our way of thinking- analyzing , structure , education and function of management in a whole!
