在1803年的名著《医学伦理学》(Medical Ethics)中,英国医生托马斯·帕茨瓦尔(Thomas Percival)将医生的角色描述为“希望使者,患者福音”,指出有时他们应该隐瞒警示信息不让患者知道。患者的生命,帕茨瓦尔写道,不但可能因为医生的行动而缩短,也可能因为他的言辞和方式而缩短。

加拿大医生威廉·奥斯勒(William Osler,诗人惠特曼的医生)也对希望的治疗效果深信不疑。事实上,一位传记作者写道,他的“可靠但有时并无保证的乐观”是他最杰出的特色。在1958年的医学伦理和法律教材上,另一位名医写道,“隐瞒一些情况在临床上常常能收获奇效并且符合患者的利益。”
